Why Compare Apples to Guanabanas?

I always tend to “cringe” when I read about comparisons between Health IT, medical devices and other healthcare related software with respect to other verticals (e.g., finance, insurance, retail).

Why compare?

Saying that the financial industry is many “light years” ahead is extremely inaccurate. Comparing the processing of debits, credits, interests, etc. with medical information and imagery is like comparing apples to guanabanas (a tropical fruit found…

Bundled Payments for Care Improvement initiative announced by CMS Center for Innovation

Many health care provider organizations have not been overly eager to jump onto the Accountable Care Organization (ACO) bandwagon, citing high startup costs and uncertain returns on investment given the complexity of the program.  Well, recently, the CMS Center for Innovation has announced the…

Software Customer Support (Large) in 4 steps

The glitz and the promises have passed and you now have an EMR/EHR launch date 3 months hence. How do you configure your internal customer support to meet your users' needs.

1. Vendor Support

Collaborate with the vendor. This means that you know what they are committing to you as a customer and how much you have to commit. In most cases, the vendor will try to offer the fewest resources and hoping that you, the customer will take up the large part of the support. In…

EMR/EHR Software Acquisition - A Tale of Duality

Ever since the California Department of Motor Vehicle computer project nightmare in the 1980's, doubts linger as to the veracity or trustworthiness participants in any large-scale information technology purchase. A duality exists that is neither spoken nor reported.  Key individual decision makers or experts often have hidden motives and may vie for the attention of either party (vendor or…

An Innovative EMR? Is it a misnomer?

Can something as mundane as an electronic medical record (EMR) be glamorous or glitzy? Can it match the pizzazz of a new smart phone or the graceful lines of a modern sports car? Probably not. A swirling array of regulations such as HITECH, ARRA, meaningful use and HIPAA compliance are masters that vendors must play to before any customer concern or complaint.

Clayton Christensen, the renowned author of innovation, concludes that innovation is possible in any kind of business or…

HIT incentives in Massachusetts: Less carrot, more stick

Health care providers all over the country are all worked up because they say that the federales' regulations on meaningful use of certified EHRs go too far, too fast. They should be glad they're not in Massachusetts, where EHR use will soon be required as a condition of licensure of physicians, hospitals and community health centers. The word got out, thanks to Secretary of Health and Human Services… Continue

EMR Software - Enigma and Dilemma

Electronic medical/health record (EMR) software is supposed to work as advertised. It is supposed to make the doctor work more efficiently and productivity. When EMR software doesn’t work, doctor productivity, bill collection and insurance reimbursement suffer. How software is produced may determine these failures.

EMR software, like most software, is designed by engineers and used by non-engineers. Bridging the two groups is critical to…

EHR Concerns

From discussions I have had with physicians it seems that their main impetus for implementation of an EHR system is to garner stimulus funds. At this point in time I am trying to steer them from this line of thought. Given the technical considerations and the disruption and re-invention of workflow we as consultants certainly have our work cut out for us to educate our clients on the potential…

The Ethical Dilemma of the EMR World

Ethics and professionalism are bywords that are not taken lightly by the medical care professions. That kind of allegiance is expected. It is ingrained early on by training and by work experience.

The EMR market, a child of the world of technology but joined at the hip to medicine, has brought together two different views or connotations of ethics and professionalism. On the one hand the technology vendor plays in the rough and tumble world of high technology where the mantra is to… Continue

HL7 CDA CCD vs. ASTM CCR ...

Lately there has been a lot of discussions whether CCR will fade out and HL7 CDA CCD emerge as the victorious champion.

Why does there have to be a winner?

In my opinion they are both useful and each have their intended audience.

Actually, we should be thinking more about developing solutions that can aid in

converting from one document type to another rather than expecting one

or the Other to disappear.

CDA CCD was derived from CCR but modeled… Continue

EMR Decision Making - Making it easier

One of the tenets of change is trying to predict and manage it. For instance, an EMR (electronic medical record) purchase or adoption isn't strictly a medical or a technical choice. In fact it is also a social program that not only tries to achieve social outcomes but also social impacts.

In the EMR RFP steering committees and evaluation panels that I have attended, the social outcome and impact has been given minimal voice, but I believe that underlying that relative silence is an… Continue

Check out John Glaser, Partners CIO, in a Podcast Interview with David Harlow on HealthBlawg

What does a large health system CIO worry about if his system is already fully up to speed in the day-to-day use of EHRs? Using them in ways that improve communication of information across a diverse group of clinicians, and that enable the integration of additional interesting and useful data as time goes on -- such as the integration of genetic testing data into the diagnostic and treatment logic built into the EHR.

John Glaser explains how Partners uses its EHR system to leverage… Continue

EMR Success Depends On Process Optimization

EMR Success Depends On Process Optimization

A large percentage of practices (more than 90%) who adopt process optimization techniques when implementing EMR, will find adoption to be successful and return on investment to be substantial. This is factor can limit the practice exposure to risk.

AARA will give Medicare incentive payments for up to five years to physicians who are “meaningful users” of certified EMR technology. According to the Healthcare Information and… Continue
